Der Altar wurde von Francesco Sassetti für eine private Kirche in Varramista (Palaia) bei Pisa anlässlich der Geburt seines Sohnes Cosimo in Auftrag gegeben. Die Eckpfeiler der Predella tragen das Familienwappen. Das Hauptpanel zeigt eine sacra conversazione: zwei stehende Heilige Flanke eine Madonna auf Wolken sitzen. Links und rechts von Marias Kopf erscheinen zwei Engel an beiden Seiten aus Wolken. Die drei Reliefs auf der Predella beziehen sich thematisch auf die Figuren über ihnen im Hauptpanel: auf der linken Seite erhält der Heilige Franziskus die Stigmata, in der Mitte Maria und Joseph knien in Anbetung vor dem Christuskind, und auf der rechten Seite wird eine Figur mit einem Apothekerglas und einer Märtyrerpalme – wahrscheinlich St. Cosmas – enthauptet. Cosmas wurde während der Verfolgung der Christen unter Diokletian zum Tode gebracht.
