"Der rote Beret auf seinem Kopf verspricht dem medizinischen Beruf von Cuspinian: "Medicus rubras fert corpore vestes" (Ein Arzt trägt rote Kleidung). Landschaft wirkt als Echokammer für mentale Zustände und stellt als solche einen Makrokosmos dar, in dem der Individuum oder Mikrokosmos seine emotionale Welt reflektiert. Vielleicht erklärt dies die Haltung von Cuspinians Kopf. Natürlich kann seine Pose dazu bestimmt sein, dem Humanisten zu zeigen, der noch über ein Buch nachgedacht ist, das er jetzt vor ihm geschlossen hält, seine linke Hand - die zwei Ringfinger aussetzt - auf seinem Deckel ruht. Sein leicht angehobener Kopf kann jedoch darauf hinweisen, dass er zuhört. Das Hörmotiv kann sich auf ein Stück Neoplatonisches Schreiben von Marsilio Ficino beziehen, das in humanistischen Kreisen besonders beliebt war: ""Es ist durch unsere Ohren, dass melodische Harmonien und Rhythmen in unsere Seelen eintreten, uns bestrafen und inspirieren, unsere Geister sofort zu heben und in den Tiefen unseres Wesens auf solche göttliche Musik nachzudenken.""
