Ein aufwändig geformter Altar wurde 1480 von der Milanese Confraternity der Unbefleckten Empfängnis für ihr Oratorium in San Francesco in Auftrag gegeben. Ein neuer Vertrag wurde im Jahre 1483 mit Leonardo und den de Predis Brüdern erstellt: Eine zentrale Tafel sollte allein von Leonardo bemalt werden, und es gab zwei Seitentafeln, die Engel singen und Musikinstrumente spielen. Zwei Gemälde von Engeln (An Angel in Green with a Vielle and An Angel in Red with a Lute) von Künstlern beeinflusst von Leonardo, sind zweifellos diejenigen für das Altarbild.
