Ce panneau Léonardesque vient de l'église de San Sepolcro, qui fait maintenant partie intégrante de la Veneranda Biblioteca Ambrosiana. Les musiciens putti applaudissent l'Enfant Jésus et Saint Roch se joignent à l'adoration de l'Enfant. Le saint est identifié par son bâton de pèlerin et sa cape, par la blessure sur sa jambe causée par la peste, et par le chien avec le pain. Puisque le saint a été particulièrement invoqué en période de peste, il pourrait être raisonnable de penser que la peinture est en quelque sorte liée à la grande épidémie qui a frappé Milan entre 1524 et 1529.
