In un paesaggio classicista idealizzato, San Carlo Borromeo si trova accanto ad un enorme sarcofago marmoreo raffigurante in bassorilievo un culto dei pastori. La performance era basata su un dipinto attribuito a Rafael da c.1635. Borromeo indica il sollievo mentre affronta due uomini, tra cui un diacono. Un uomo storpio siede vicino a lui per il sarcofago. A sinistra in primo piano, la Sacra Famiglia cavalca con il Bambino Cristo sugli asini, accompagnato da un giovane. Nel mezzo un ragazzo cieco e una ragazza cieca attraversano un flusso d'acqua. Proprio sul secondo piano, un uomo riccamente vestito dà le alms a un gruppo di mendicanti.
