После вспышки чумы в городе Палермо, жители сообщили, что Сент-Розали показал местоположение ее смертных останков. Реликвии стали символом защиты от болезни и напоминанием о тех, кто погиб от нее. Ван Дайк сделал несколько изображений святого во время своего пребывания в Палермо и по возвращении в Антверпен. В этой версии, Святая Розали на коленях рядом с ее останками, когда она просит о благополучии своего города, изображенного на заднем плане.
