Dopo uno scoppio della peste nella città di Palermo, i residenti hanno riferito che Santa Rosalie era apparso rivelando la posizione dei suoi resti mortali. Le reliquie divennero un simbolo di protezione dalla malattia, e un promemoria di coloro che ne erano periti. Van Dyck completò diverse immagini del santo durante il suo soggiorno a Palermo e al suo ritorno ad Anversa. In questa versione, Santa Rosalie si inginocchia accanto ai suoi resti, mentre si supplica per il benessere della sua città, raffigurata sullo sfondo.
