Secondo il Vangelo di San Matteo, dopo aver ascoltato i saggi della nascita di Gesù, il re Erode ordinò che tutti i bambini di Betlemme sotto i due anni fossero uccisi. Bruegel ha stabilito la storia come atrocità fiamminga contemporanea in modo che i soldati indossano l'abbigliamento distintivo dell'esercito spagnolo e dei loro mercenari tedeschi. L'artista ha anche tratto la sua esperienza dell'inverno eccezionalmente grave del 1564-5 per descrivere un villaggio coperto di neve, con ghiaccioli appesi dai tetti e lo stagno in primo piano densamente congelato sopra.
