Agostino Perrocchio (1720-1799), notaio, amico di Giuseppe Macchi, altro grande benefattore dell'ospedale, nominato erede universale di un lascito cospicuo, l'Ospedale Maggiore, che aveva già beneficiato durante la sua vita. La commissione del ritratto è affidata a Anton Francesco Biondi, pittore, che nel corso di trent'anni dipinge 12 ritratti per Quadreria Ospedaliera e costruisce un bellissimo ritratto
